Medical History

Please completely fill out and answer the medical history form. This form is required by law in order for your child to be enrolled with Little Ambassadors’ Academy or Ambassadors Adventure Camp and to begin with our program.

Medications kept on-site must be in original packaging. Prescription medications must also have the prescription label attached. We cannot accept expired medications of any kind.
